How to use SAFO
Safe and Found Online is an online version of the Herbert Protocol form, allowing a simple process to complete the form and update the information should it change. We’ve designed it in partnership with police forces across the country and people living with dementia and their carers.

The form can be completed by a person living with dementia, an informal or professional carer or other key person connected to the person at risk of going missing and, while it is preferable for the whole form to be completed, it can be partly filled in and completed later when more information is available. The questions are based on experience of what is needed to find a missing person as quickly as possible and all the questions are important, so please take the time to do this as it can reduce worrying and distress when a person goes missing and potentially save lives.
